Wear loose or short-sleeved shirt.
Bring proof of current insurance (card/e-card).*

*Kaiser Permanente student-specific plans are not eligible. (Restriction by Kaiser, who provide shots at their local locations instead.)
DU SHIP and most other insurance plans accepted.
Faculty and staff on Kaiser, Cigna, or other plans are eligible.

FYI, if on-campus clinics don't work for your schedule, most insurance plans cover flu (and COVID) vaccine via retail pharmacies, such as the Safeway and Walgreens locations near campus (Evans Ave and Downing St).

PRE-PRINT/COMPLETE YOUR FORMS: We have attached PDFs of English and Espanol Immunization forms; you can download, PRINT and PRECOMPLETE one to speed up your clinic visit. Fill in A) Top 2 sections (don’t worry about Primary Care Provider); B) Signature and date (about 5/6th of way down); AND C) On back, please note the following from your insurance e/card: Name of insurance company (Cigna, Tricare, DU student Anthem, etc), and any non-dollar amount identifying info: Member/subscriber #, Group#, Policy#, RxBIN#, RxPCN#, etc. include label for what the number is ("Group# 123456"). Every plan has slightly different labels and info; we need all the identifying details in order to bill your insurance. (In-store systems prompt for plan-specific details; but we’re offline for today, so need all possible.) Do NOT need phone #s, co-pay ($) amounts, etc.
